If you would like to make Strand Urgent Care your primary care provider, please call us for an appointment.
Please download and complete the New Patient Packet, Medical Records Release, and HIPPA and Privacy Policies forms from the Downloads section below.
Arrive at least 15 minutes before your scheduled appointment.
Please have the following items with you for your first appointment:
1 - Insurance Card
2 - Valid Picture ID
3 - Credit Card to place on file
4 - Complete Medication List
5 - Completed New Patient Paperwork
Prior to your appointment, please check your insurance so you are aware of any co-payments or deductibles required at the time of your visit.
You are encouraged to download and complete the New Patient Paperwork before you arrive.
We accept most major insurances, including Medicare, Medicare Advantage plans, Medicaid, Blue Cross/Blue Shield, United Healthcare, Cigna, Aetna, and many others.
Please call the customer service number on the back of your insurance card for details regarding your specific plan, and to see if we are in network with your plan.
If we are unable to verify your insurace is active and in good standing, you will be considered self pay and would need to pay for your visit in full.
Co-payments, co-insurance, and any outstanding balances will be due at the time of check in.
We accept all major credit cards, debit cards, personal check, money order, cashier's check, or cash.
We require a credit card on file for your convenience to pay for any outstanding balances not covered by your insurance.
If your insurance has not responded to a claim after 90 days, we reserve the right to transfer all associated financial liability to the patient or guarantor.
Return check fee is a $30 charge to your account.
Unpaid patient accounts are subject to third party collections and/or legal proceedings.
